A Guide to the Care we Provide..

The information on the following pages is provided to assist with the selection of services and merchandise which you might find most meaningful in honoring the memory of your loved one. It is presented in a format, prescribed by the Federal Trade Commission, to standardize the manner in which information is presented to those making decisions regardless of where the information is obtained.

We believe it is important to recognize that the selection of a particular type of service and merchandise, and for that matter, a service provider, entails more than reviewing a list of available items. Our belief is founded in the philosophy that each person we serve should be cared for in the manner in which we would wish to be cared for in the same situation.

Recognizing that each person is unique with unique needs we strive to provide care which honors this uniqueness. We believe that our ability to provide care at all levels of need and do so with compassion and professionalism sets us apart from other caregivers in our profession.

We also believe that providing service in the community necessitates participation in community activities and support of community entities and functions. For this reason we support our communities’ churches, schools, libraries, arts, fire department, rescue squad, health care facilities, youth activities, historic buildings and events, fraternal organizations, charitable foundations and many more activities with a portion of the operating income we receive from providing service.

As you consider the information below we encourage you to ask questions about the things which are most meaningful to you and how our care might assist in creating an experience which you find supportive and memorable.

The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean that you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

This list does not include prices for certain items that you may ask us to purchase for you such as cemetery or crematorium charges, flowers and newspaper notices. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected.
  1. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF

  2. Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service, arrangement conference with family or responsible party; arrangement of services/disposition; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary notices; staff assistance prior to and following the funeral including coordination with those providing other portions of the funeral, e.g. cemetery, crematory and others. Also included in this charge is overhead expense relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative costs and equipment and inventory expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charge for direct cremations, immediate burials, and forwarding or receiving remains.)

  3. EMBALMING

  4. Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.


  5. OTHER PREPARATION OF THE BODY

    • Restoration when necessary per hour
    • Special cosmetology per hour
    • Hair dressing
    • Special care of autopsied remains
    • Washing and disinfection of remains when no embalming


  7. ADDITIONAL SERVICES AND FACILITY FEES/USE OF FACILITIES:

  8. • Use of facilities and staff for viewing (visitation/wake/per two hour session)
    • Use of facilities and staff for private viewing by the family
    (limited to immediate family, one-half hour)
    • Use of facilities and staff for a memorial service in the funeral home
    • Use of facilities and staff for a funeral in the funeral home
    • Use of staff and equipment for a memorial service in a church or other facility
    • Use of staff and equipment for a funeral in a church or other facility
    • Use of staff and equipment for a graveside service
    • Shelter of deceased per day or portion thereof. This is a declinable charge if our facilities are not used to shelter deceased. This charge will be made on the fifth day our facility is used to shelter deceased.
    • Refrigeration of deceased

  20. FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME

  21. This charge includes removal of remains, necessary services of staff, embalming, necessary authorizations, and local transportation to airport or railhead. This charge does not include the services of staff or the use of facilities for any visitation and/or rites or ceremonies prior to forwarding of the body.

  22. RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME

  23. Includes temporary shelter of the remains, transportation of remains to cemetery or crematory and necessary services of staff. It does not include visitation, rites or ceremonies.

  24. IMMEDIATE BURIALS

  25. Our charge for an immediate burial without any attendant rites or ceremonies includes:
    • removal and shelter of the remains
    • local transportation to cemetery
    • necessary services of staff and authorizations.
    • Immediate burial with casket selected from our funeral home
    • Immediate burial with rough cut pine container
    • Immediate burial with container provided by purchaser


  26. DIRECT CREMATIONS:

  27. Our charge for a direct cremation without any attendant rites or ceremonies includes:
    • Removal of remains and local transportation to crematory.
    • Necessary services of staff and authorizations.

    ****If you want to arrange a direct cremation, you can use an alternative container.

    Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without covering). The containers we provide are card- board, rough cut pine, lined and covered fiberboard, finished pine.

    1. Direct Cremation with casket selected from our funeral home (in addition to cost of casket)
    2. Direct Cremation with least expensive alternative container
    3. Direct Cremation with container provided by purchaser

    Above costs DO NOT include crematory charges.

  28. BODY DONATION TO A MEDICAL SCHOOL OR FACILITY

  29. (Includes local transportation only-no services-all care except initial transfer provided Monday-Friday 8 A.M. - 5 P.M. - Saturday 8:30 A.M. to 12:00 Noon FULL PAYMENT REQUIRED AT TIME OF ARRANGEMENT)

    INFANT AND CHILD SERVICES:
    The loss of a child is especially difficult for family members. As a token of our support to parents in these special circumstances our fees will be reduced as noted below for professional services, facilities and local motor equipment. The family is asked to pay for cash disbursements and merchandise selected.

    MONUMENT SERVICE:
    Complete monument/memorial offerings available through Shea Monument Company.

  30. Our basic service charge includes a $6.00 fee that will be sent to the State of Vermont and set aside for the protection of funeral service customers.



  31. Funds may be available for final disposition through the Vermont Department of PATH, General Assistance Program for persons who qualify.



  32. The funeral industry is governed by laws defining professional conduct. Consumers who have inquiries or wish to obtain a form to register a complaint may do so by calling 802-828-2367 or by writing the Secretary of State at 109 State Street, Montpelier, Vermont 05609-1106.
